Give consent to my child to partake in the Academy's activities and that he/she is in good health and furthermore, consider him/her capable of taking part in the activities.

I understand that whilst the Technical Team will take every precaution to ensure that injuries are prevented at all costs, they cannot be held responsible for any loss, damage to my child's property or any seriuos injury that might be suffered by my child.

I give consent for Shooting Stars Soccer Academy to use my images taken during training for advertisement, social media and any other way they deem necessary

I agree to honour and respect the Guidlines and Policies of the Academy an I agree to ensure the safe transportation of my child, including prompt colletion of the child from training.

Payment Procedure

- Parents are encouraged to pay monthly fees by the 7th of the new month unless communicated with the Administrator. Failure to settle your child's account on timewill result in cancellation/suspension of training sessions


- All parents who pay through the EFT System or Bank Deposits are encouraged to send a proof of payment to info.sssa1@gmail.com or to our WhatsApp 067 768 8029


- This agreement will be valid until December 2025. Parents will be required to pay for their child until the end of December 2025.
